KVKK Application and Information Request Form

Pursuant to the Law on the Protection of Personal Data No. 6698 (“KVK Law”), individuals defined as data subjects (“Applicant”) are granted the right to make certain requests regarding the processing of their personal data under Article 11 of the KVK Law.

According to Article 13/I of the KVK Law, applications to be submitted to our Company, which is the data controller, regarding these rights must be delivered to us in writing or through other methods determined by the Personal Data Protection Board (“Board”).

It is important that the personal data we hold about you is accurate and up-to-date. Therefore, please notify us in case of any changes in your personal data. Pursuant to the KVK Law and the relevant legislation, regarding yourself, you may:

• Learn whether your personal data is being processed,

• Request details regarding the information we have collected,

• Learn the purpose of collection and how it is used, and the third parties to whom personal data is transferred domestically or abroad, or

• Request rectification or updating of your personal data in case it has been processed incompletely or inaccurately, or request deletion if the reasons requiring the processing have ceased to exist, including requesting notification to the third parties to whom such data has been transferred.

Within this scope, applications to be submitted to our Company in writing may be delivered to us by printing this form and writing “Personal Data Protection Law Information Request” on the envelope or in the subject line, through:

• Personal application of the Applicant,

• Notary public or registered mail with return receipt,

The rights that you may request from our Company, as the data controller, are specified in Article 11 of the KVK Law. Applications submitted to us will be evaluated within our Company according to the rights you request, and in accordance with Article 13/2 of the KVK Law, your request will be responded to as soon as possible and no later than 30 (thirty) days from the date it reaches us, depending on the nature of the request. Pursuant to Article 13/3 of the Law, our response will be communicated to you in writing or electronically.

The Company does not accept any liability for requests resulting from inaccurate or outdated information provided in the form or unauthorized applications.
